As the parents or legal guardians of the child named above, we hereby give permission for our child to participate in classes and programs at The Plein-Air Art Academy at its usual place of business at 320 Washington St Suite 302, Brighton, MA 021355, and any other locations chosen by The Plein-Air Art Academy for its programs. On behalf of our child and on our own behalf, we indemnify and hold harmless, and waive all claims against The Plein-Air Art Academy, its principals, agents, servants, and employees from and against any loss, cost, charge, claim, demand, liability, damage, medical expenses, arising directly or indirectly from any personal injury or property damage loss, which may occur to my child while on the premises of The Plein-Air Art Academy or any other location provided by The Plein-Air Art Academy and/or under instruction, supervision, or control of The Plein-Air Art Academy.
